Individual Therapy (53 minutes): $165.00 (CPT 90837)
Family Therapy (50 minutes): $165.00 (CPT 90846 or 90847)

Cancelations & Missed Appointments

I understand that life happens and scheduling conflicts and emergencies may arise. Notice to cancel an appointment is expected no less than 24 hours before the appointment. Appointments cancelled less than 24 hours in advance or no-shows are charged the full session fee of $165.

Insurance

I am in network with the following insurance plans:

• Premera
• LifeWise

For all other insurance plans, I am considered an out of network provider. If you have out of network benefits, I can provide you with a statement that you can submit to your insurance company for reimbursement.
